Contractor responsibilities:

• SERVICE TASKS: Contractor will perform the following tasks in the time and manner specified:
• Launch and maintain the Flex Website as follows:
• Within 30 calendar days of issuance of the PO, launch the Flex Website. Ensure the Flex Website contains the following as directed by the Department":
• Public information and resources for the general population;
• A password protected access portal for the CAHs;
• Flex Program information, contact information, access points to online education, links to previous recordings and broadcasts, and other reference material; and
• Any other information as directed by the Department.
• As directed by the Department, provide the appropriate webpage links and login credentials to the Department and CAH affiliates within thirty calendar days of issuance of the PO.
• As directed by the Department, provide Department specified personnel administrative and editing access to the Flex Website within thirty calendar days of issuance of the PO.
• Upon launching the Flex Website, ensure the CAHs have access to the Flex Website throughout the PO term.
• As directed by the Department or as needed, maintain and update the Flex website throughout the PO term. Document any maintenance and/or updates conducted to the Flex website and submit the documentation with the corresponding invoice.
• As directed by the Department, create an annual statewide Medicare Beneficiary Quality Improvement Program (MBQIP) by June 30 each year of the PO term Provide updates on the creation of the MBQIP to the Department upon request.
• Each month and as directed by the Department, create a MBQIP resource(s). Such resources include: priority measures, site visit TA, webinar topics, and other related information. By the end of each month, submit the MPQIP resource(s) to the Department Contract Manager for review and approval. Upon Department approval, post the approved resources to the Flex website.
• Conduct at least 12 site visits (in-person or virtually) each year of the PO term by June 30 with CAH leadership as directed by the Department and as follows:
• As directed by the Department, create a schedule for the site visits that includes the date of the site visit and identifies the CAH that will be assessed and submit the schedule within 30 calendar days of issuance of the PO. Update the schedule as needed and submit the updated schedule to the Department Contract Manager within 5 business days of the update.
• As directed by the Department, create site specific TA for each site and submit the sitespecific TA prior to conducting the site visit to the Department Contract Manager for review and approval.
• As directed by the Department, meet with the Department to review the site-specific TA created. Create a meeting agenda for each meeting and submit a copy of the agenda with the corresponding invoice.
• Identify additional performance analysis criteria as needed prior to conducting the site visit and submit any identified performance analysis criteria prior to the site visit to the Department Contract Manager for review and approval.

Experience Required:

• A minimum of five years of experience in data collection and/or data analytics.
• Minimum of five years of experience working with rural providers and familiarity with health and human services (e.g., rural hospitals, rural health clinics) and/or rural stakeholders (e.g., rural health networks, local health department in rural counties, Area Health Education Centers (AHECs) in rural counties).
• A minimum of five years working with grant programs.
• Documented experience working with health and human services programs, providing technical assistance and data analysis.
• Documented ability to deliver comprehensive work plans, implement assessments, and the ability to deliver training and detailed technical assistance.
• A minimum of five years proven familiarity with quality improvement strategies, culture of change, and process improvements.
